Trip leaders must fill out this form before each trip (one form per trip). The form contains information about where the trip is going, when, who is going on the trip, and other information that would be beneficial to have in the event of an emergency.

Please fill out the Trip Leader Form at least 3 days before your trip departs.

 General guidelines for trip leading:

  1. All trip participants including Trip Leaders need to fill out the Waiver Form.
  2. All trip participants including Trip Leaders need to complete the Student Travel Waiver
  3. Trip leaders are responsible for ensuring the completion of travel waivers for all participants, if the waivers aren’t done you won’t be able to pick up keys from Fleet Services. ( As a reminder, Fleet Services is open 8:00 AM until 5:00 PM Mondays through Fridays during the term. )
  4. Get plenty of rest the night before your trip leaves.
  5. Respond to the needs and abilities of your group.
  6. Remember: you’re there to have fun!
