Hauntcert Guest Registration Form 2022

8th Annual Hauntcert, Saturday, October 29th:

• Currently enrolled students may register one (1) guest.
• Guests must be registered and complete an online Waiver of Liability, Assumption of Risk, and Indemnity Agreement by 5:00pm on Friday, October 28th. Waiver will be sent to your guest via email using the email address you provide in the form below. Note, it’s the responsibility of the host that the guest completes the waiver prior to the deadline.
• Guests must be 18 years of age or older.
• Guests must present a photo ID at entry.
• Guests must arrive and be accompanied by their host at all times.
• As the host, you are responsible for the actions of your guest.
