GSC Authorized Name Change Form
Use this form to notify Campus Services of name changes for students, faculty, and staff who are transgender and gender non-binary and wish to have a new OneCard printed with their first name of choice.

Authorized faculty and staff may charge dining transactions to their department with their OneCard.
