Congratulations on your new employment with Carleton College! We ask any new employee who is not a student or food service worker to complete the paperwork listed below and turn in the completed forms at the New Hire appointment scheduled during the first days of employment. All forms must be completed unless otherwise noted. If you have questions regarding the completion of the forms or are not able to complete/print the forms, do not hesitate to contact Human Resources at 507-222-4830.
- New Employee Information Form
- This form is to establish a record of you as an employee for payroll and other employment systems at Carleton College. Please complete this online form at least one week prior to your start date.
- W-4 Form-Federal
- Must be completed for tax purposes.
- W-4 State
- The Minnesota W-4 only needs to be completed if you want to claim a different tax rate than what you have listed on your federal W-4 form or if you are claiming exempt status.
- I-9
- Please complete Section One of the I-9 form. You will also need to bring your Employment Verification documents to your HR meeting. Documents required are listed on page 9 of the I-9 form. If you are a alien authorize to work in the United States, you will need your I-94 admissions form .
- Employee Handbook Acknowledgement Form
- For all staff must read and sign. Faculty do not need to complete this form.
- Direct Deposit Form
- Payroll is paid through direct deposit only.
- You may have upto 3 accounts, you must provide the following information for each account
- Bank Routing number
- Bank Account number

- Child Support Obligation Disclosure
- Motor Vehicle Report & Record Release
- Required of all employees as part of the Carleton College’s Transportation Program. A driver’s license is required for completion of this form. If you are not a licensed driver, please disregard this.
- Carleton Community Resource Program
- Program to provide support from staff outside of your department to help you connect with the Carleton Community. Faculty or temporary staff do not need to complete this form.
- Responsible Technology Use Agreement
- Family Education Rights and Privacy Act (FERPA)
- Please read through the tutorial and then click on the appropriate notification at the end of the tutorial to complete the FERPA Acknowledgment.
- Sexual Misconduct Response Online Training
- Please register and complete the Haven training.
- Health Insurance Portability and Accountability Act (HIPAA) Training (For new Supervisors only)
- If you will be a supervisor in your new role, please read through the tutorial and training. Complete the HIPAA Receipt and Acknowledgement at the end of the training.
