Government Notices
Carleton is responsible for providing the following Government notices to employees upon hire and annually thereafter.
- Children’s Health Insurance Program (CHIP) Notice: For children who meet the eligibility criteria, CHIP provides low-cost health coverage to children in families who do not qualify for Medicaid. This notice provides instruction on how to apply for CHIP coverage.
- HIPAA Portability Notice – Special Enrollment Rights: This notice provides information about your options for maintaining certain insurance benefits when you have a change in employer or when you retire.
- Health Information Portability Accountability Act (HIPAA) Notice of Privacy Practices: This notice informs employees about the privacy rights regarding their personal health information and the privacy practices of the health plans and health care providers.
- Initial Consolidated Omnibus Budget Reconciliation Act (COBRA) Notice: This is a federal law that gives workers and their families the right to continue group health benefits for limited periods of time under certain circumstances. At Carleton, eligible benefits include medical insurance, FSA, life insurance, and dental insurance.
- Marketplace Notice of Exchange: The Affordable Care Act (ACA) requires employers to inform their employees (regardless of their medical plan enrollment) about their options for health insurance coverage through various sources, including health insurance exchanges (known as insurance marketplaces). In Minnesota, our state’s marketplace is MNSure.
- Medicare Part D Creditable Coverage Notice: This notice confirms that Carleton College’s health and prescription drug plans provide creditable coverage. Creditable coverage means that our health and prescription drug coverage meets or exceeds Medicare Part D coverage.
- Notice of Rights Under the Women’s Health and Cancer Rights Act of 1998: This notice explains the benefits provided to women while dealing with breast cancer.
- Statement of Erisa Rights: This notice provides information about your rights to review and receive (upon request) plan documents and benefit information.
- Newborns Act Disclosure (Federal): This notice provides information about your rights regarding duration of hospital stays in connection to childbirth.
