Government Notices
Carleton is responsible for providing legal government notices to employees upon hire and annually thereafter. The notices are contained in one document: Annual Legal Notices. Provided below is a listing of the respective notices contained in this document with a brief description of each.
- The Women’s Health Cancer Rights Act of 1998 (WHCRA): This notice explains the benefits provided to women while dealing with breast cancer.
- Newborns Act Disclosure (Federal): This notice provides information about your rights regarding duration of hospital stays in connection to childbirth.
- HIPAA Portability Notice – Special Enrollment Rights: This notice provides information about your options for maintaining certain insurance benefits when you have a change in employer or when you retire.
- Statement of Erisa Rights: This notice provides information about your rights to review and receive (upon request) plan documents and benefit information.
- Health Information Portability Accountability Act (HIPAA) Notice of Privacy Practices: This notice informs employees about the privacy rights regarding their personal health information and the privacy practices of the health plans and health care providers.
- Medicare Part D Creditable Coverage Notice: This notice confirms that Carleton College’s health and prescription drug plans provide creditable coverage. Creditable coverage means that our health and prescription drug coverage meets or exceeds Medicare Part D coverage.
- Children’s Health Insurance Program (CHIP) Notice: For children who meet the eligibility criteria, CHIP provides low-cost health coverage to children in families who do not qualify for Medicaid. This notice provides instruction on how to apply for CHIP coverage.
- Marketplace Notice of Exchange: The Affordable Care Act (ACA) requires employers to inform their employees (regardless of their medical plan enrollment) about their options for health insurance coverage through various sources, including health insurance exchanges (known as insurance marketplaces). In Minnesota, our state’s marketplace is MNSure.
We are also including the Initial Consolidated Omnibus Budget Reconciliation Act (COBRA) Notice: This is a federal law that gives workers and their families the right to continue group health benefits for limited periods of time under certain circumstances. At Carleton, eligible benefits include medical insurance, FSA, life insurance, and dental insurance.
