Medical Leave
Process for Returning from a Medical Leave
Medical leaves allow students up to three terms of non-academic time away from campus. You may return when we have appropriate medical verification of your readiness to return, and have the support of Student Health and Counseling (SHAC). Please follow this checklist of items needed prior to your return:
- Release of Information Form allowing SHAC to communicate with your medical professional.
- Release of Information Form allowing the Dean of Students Office to communicate with SHAC.
Submit the Release of Information Forms to Student Health and Counseling (shac@carleton.edu)
- Provider Questionnaire (for your medical provider to complete)
- Questionnaire for returning from medical leave due to physical health.
- Questionnaire for returning from medical leave due to mental health.
Submit the Questionnaire to Student Health and Counseling (shac@carleton.edu)
- Personal Statement of your readiness to return submitted to Dean of Students Office (dosoffice@carleton.edu), include the following
- How you have used the time away to address the reason for your leave.
- What your current educational goals are and how Carleton fits into those goals.
- What steps you will take to help ensure your success when you return.
- Set an appointment to speak with Associate Dean Sindy Fleming regarding your request. To set up an appointment call the Dean of Student Office at 507-222-4075.
Deadlines for submitting these documents in preparation for your return as follows:
Fall Term: August 1
Winter Term: December 1
Spring Term: February 20
Student Health and Counseling, in consultation with your doctor, will assess your readiness to return and arrange for necessary support if you return. Return from a medical leave must be approved by the Dean of Students Office in consultation with Student Health and Counseling. The review and decision process may take up to two weeks. Upon returning to campus, you need to meet with your class dean during the first week to discuss your academic transition.
Military Leave
Process for Returning from a U.S. Military Leave
Students petitioning to return to the College from U.S. military service, who were in good academic standing at the time of their leave, will typically be granted readmission.
To request to return to the College please follow this checklist of items needed prior to your return:
- Personal Statement of your readiness to return submitted to Dean of Students Office (dosoffice@carleton.edu), include the following
- How have you used the time away to address the reason for your leave.
- What your current educational goals are and how Carleton fits into those goals.
- What steps you will take to help ensure your success when you return.

Personal Leave

To request to return to the College please follow this checklist of items needed prior to your return:

Withdrawal From the College
Readmission Petition Process
To petition for readmission to the College please follow this checklist of items needed prior to your return. You need to have left in good academic standing. If you attended another institution during the leave, you will need to apply for readmission through the transfer process.

- What steps you will take to help ensure your success when you return.
